Please present your insurance card at each of your appointments. The front office staff will copy your cards and any other information pertinent to your insurance coverage. Insurance plans vary widely, so it is important that you contact your insurance company and verify your eligibility and coverage as well as applicable co-insurances.

Q: Will my insurance cover this procedure?
A: Your insurance coverage is based on criteria outlined in your insurance plan coverage document. It is best to contact your insurance company if you are unsure about whether they will cover the test. Even if your insurance plan covers a test, you will be responsible for any co-payment, co-insurance or deductible amount specified by your insurance plan.
Q: Will I have an out-of-pocket or co-insurance expense for this procedure?
A: As this is dependent on your specific insurance coverage, please check with your insurance company to determine exact out-of-pocket expenses for the procedure.
Q: Is Diagnostic Imaging Northwest a participating provider?
A: Diagnostic Imaging Northwest is a preferred provider with all the major and most other insurance companies. Please check with your insurance provider to ensure that your procedure will be covered under your plan.
Q: Do I need an insurance authorization for my procedure?
A: Several insurance plans require pre-authorization for certain types of radiology tests; typically MRI, CT, PET Scans and Nuclear Medicine studies. It is the responsibility of your referring physician to contact your insurance plan, obtain the necessary authorization and then provide the authorization number to Diagnostic Imaging Northwest.
